How to refer

All referrals to Willow Wood Hospice services will be reviewed in the first instance by our Care Co-ordinator, who may contact referrers and/or patients and those closest to them for more information.

Working as a single point of contact for Hospice referrals, our Care Co-ordinator, Paula (pictured), is responsible for reviewing, gathering information and ensuring the referrals are shared with the right clinical team for triage purposes.

Our Care Co-ordinator will also provide updates on status of the referral, whilst navigating services to ensure that people get the right support, in the right place and at the right time.

Who can be referred?

Healthcare professionals may refer any patient to the Hospice with a life-limiting illness (e.g. Cancer, COPD, Heart Failure, Motor Neurone Disease, Dementia).

Referrals are welcomed for patients residing in the Tameside and Glossop area.

Who can refer?

Any healthcare professional such as: GPs, Macmillan Nurses, Hospital teams and professionals, District Nurses, Long Term Condition Teams.
